Referral Process Information

1. Ask your doctor to complete and send us the Allevio Referral Form.

2. If you are a Case Manager looking to refer a patient or work with us
Please complete the Referral forms as well and send us an email (referrals@allevioclinic.com) to confirm receipt and discuss your patient’s challenges and goals.
3. Complete the Allevio Pre-Consult Questionnaire
Please complete digitally and email to Referrals@AllevioClinic.com or print and complete manually as required.
4. Please include any relevant notes, images or documentation with your Referral
It is very important that any referral come with all relevant notes, images and documentation that the Allevio team may need to understand the patient’s needs. We are unable to book initial consultations without these forms.

6. Allevio Assessment and Initial Consultation Process
Your Allevio practitioner will make their own assessment of patient conditions and will work with the Family Doctor to create the best treatment plan possible. Allevio will contact you once we have your referral form to ask any further questions and set up an initial consultation appointment.
*Patients require a family physician, as we are unable to dedicate resources to prescribing maintenance opioid therapy at this time.
