Allevio’s Rapid Access Collaborative Assessment program provides patients with personalized and comprehensive care focused on their road to recovery.
The program is designed for patients who want a quick, comprehensive, and 360° look into their condition, clear answers, and an understanding of their path forward.
The Patient Navigator app gives patients telehealth pain management, direct access to their Patient Navigator, and clear guidance.
Dedicated support of a Patient Navigator answers follow-up questions, address medication concerns, and ensures that patients have an expert at Allevio who is willing to guide them through their journey.
How Our Rapid Collaborative
Intake Works
We work with patients to determine their diagnosis, prognosis, and on an optimal treatment plan that’s right for them.
The team is specifically chosen for each patient and may include a combination of Allevio’s physicians, nurse practitioners, occupational therapists, physical medicine specialists, psychological support professionals, and physician assistants as part of the package.
Describe your symptoms
The initial coordination meeting with the Patient Navigator will consist of preliminary questions and the patient’s thoughts to the care team helping them.
Get an assessment
Next, the patient will be connected with their team for assessments. This team can be virtual or in-clinic.

Meet with a Patient Navigator member for the preliminary intake session.
The meeting will consist of questions for the patient and information about the care team.

After the preliminary intake session, you may get in touch with a physician assistant for further pre-consultation discussions while we begin your assessments.
Receive a second opinion from dedicated experts
The Second Opinion program allows patients to receive a one-time, independent consultation with an Allevio physician assistant, nurse practitioner, physician, or trusted Allied Health Provider.
